Eligible state and local governments receive annual block grants for community development and affordable housing from the U.S. Department of Housing and Urban Development (HUD).
The consolidated planning process serves as the framework for a community-wide dialogue to identify housing and community development priorities that align and focus funding from the HUD formula block grant programs. These grants include the Community Development Block Grant (CDBG), the HOME Investment Partnerships Program (HOME), and the Emergency Solutions Grant (ESG).
In order to determine the most pressing needs and develop effective, place-based market-driven strategies to meet those needs, HUD requires grantees to develop a Consolidated Plan.
- Assessment of Fair Housing (AFH). All local governments that receive formula funding from Community Development Block Grants (CDBG), HOME Investment Partnerships Program (HOME), or Emergency Solutions Grants (ESG) are required to analyze fair housing issues and related contributing factors in their jurisdiction and region and set fair housing priorities and goals through an Assessment of Fair Housing (AFH) report. Those goals must inform subsequent housing and community development planning processes. The U.S. Department of Housing and Urban Development (HUD) must receive the AFH prior to the development of the Consolidated Plan.

- The Consolidated Plan. The Consolidated Plan describes the jurisdiction’s community development priorities and multi-year goals for using CDBG, HOME and ESG Grants. Priorities and goals are based on an assessment of housing and community development needs, an analysis of housing and economic market conditions and available resources.

- The Annual Action Plan. The Consolidated Plan is carried out through Annual Action Plans. Action Plans are prepared by entitlement grantees provide a concise summary of the actions, activities, and the specific federal and non¬federal resources that will be used each year to address the priority needs and specific goals identified by the Consolidated Plan.
Annual Action Plan: 2015
- Consolidated Annual Performance and Evaluation Report (CAPER). Local governments that receive CDBG, HOME, and ESG funds report all activity occurring through the last day of the program year in the CAPER. Grantees are required to submit their CAPER to HUD within 90 days after the end of their annual program year (Des Moines program year runs from January 1 to December 31 of a given year).
- HUD Annual Income Guidelines for CDBG/HOME/ESG and NSP Programs. The AMFI data is updated annual and includes the Des Moines/West Des Moines Metropolitan Statistical Area (MSA). The MSA contains the following areas: Dallas County, IA; Guthrie County, IA; Madison County, IA; Polk County, IA; and Warren County, IA.
Income estimates use a combination of data from the Bureau of the Census American Community Survey and Bureau of Labor Statistics earnings and employment reports.

- HUD Annual Low-Moderate Income Summary Data (LMISD) Map: CDBG Des Moines. This map contains summary estimates of the number of persons that can be considered Low, Low to Moderate, and Medium income persons according to annually revised income limits. Data are provided at the Census Tract-Block Group level. The statistical information used in the calculation of estimates identified in the data set is derived from the Bureau of the Census American Community Survey and other HUD data resources for the CDBG grant program. In 2015, 53.9% of the total population of Des Moines meets the area Low-Moderate Income thresholds.

Community Development Block Grant (CDBG) Program
The Community Development Block Grant (CDBG) Entitlement Program provides annual direct grants on a formula basis to states and eligible metropolitan cities (called "entitlement communities") to develop viable urban communities by revitalizing neighborhoods, expanding affordable housing, and/or improve community facilities and services, and by expanding economic opportunities, principally to benefit low- and moderate-income persons.
HOME Investment Partnerships (HOME) Program
The HOME Investment Partnerships Program (HOME) provides annual direct grants on a formula basis to states and eligible metropolitan cities that are used - often in partnership with local nonprofit groups - to fund a wide range of activities including building, buying, and/or rehabilitating affordable housing for rent or homeownership.
Emergency Solutions Grant (ESG) Program
The Emergency Solutions Grant (ESG) Program provides annual direct grants on a formula basis to states and eligible metropolitan cities.
The ESG program provides funding to:
- Engage homeless individuals and families living on the street;
- Improve the number and quality of emergency shelters for homeless individuals and families;
- Help operate these shelters;
- Provide essential services to shelter residents;
- Rapidly re-house homeless individuals and families; and
- Prevent families and individuals from becoming homeless.
All ESG entitlement communities must consult with the Continuum of Care operating within the jurisdiction in determining how to allocate ESG funds.
