On July 12, 2004, the City Council of Des Moines approved a Franchise Fee Economic Development Grant program whereby businesses classified as "manufacturers" under the North American Industry Classification System may receive grants for a portion of the amount of the franchise fee on their future gas and electricity purchases. For those businesses that qualify and complete the procedural steps explained below, grants will be paid by the City beginning in December 2004, and will be applied automatically to their gas and electric bills beginning in March 2005

STEP 1 - Determining Eligibility

The definition of manufacturing is established by the North American Industry Classification System ("NAICS"). To determine if your business meets the definition of manufacturing, please consult the web site at http://www.census.gov/epcd/naics02/naicod02.htm , and click on "manufacturing". If your business is considered a manufacturer by NAICS, note the code number. Only those meters specifically tied to your manufacturing operations are eligible for the grant. Then proceed to complete the form below.

STEP 2 – Filling Out the Form

Definition of fields:

• Business Name – Name of the company billed by Mid American Energy.
• Business Address – Address of the company billed by Mid American Energy.
• Contact Person – The person to be contacted with questions concerning your grant.
• Product Manufactured – A brief description of what your business manufactures.
• NAICS Code – The code of your manufacturing business, found on the website in Step 1.
• Mid American Energy Account Number – The account(s) for which you are requesting a grant.
• Service Address – Address of the account where service is located.
• Service Month – Billing month for which you are requesting grant.
• Total Franchise Fee Paid for Month – The total amount of franchise fee paid for month.
• Grant Amount – Bills for service during December 2004 through June 2005, 2/3 of the amount on the line above.

  Bills for service for July 2005 and after, 2/5 of the amount on the line above.
• Number of Employees - The average number of full-time and part-time employees employed by your business in Des Moines on the first day of each of the past six calendar months.

STEP 3 – Submission of Form
