AGENDA HEADING:

 

Approval of 2008 Supportive Housing Program (SHP) Renewal Grant Agreements with the United States Department of Housing and Urban Development (HUD).

 

 

SYNOPSIS:

 

Recommend approval to renew the SHP grant agreements for Primary Health Care’s (PHC) Street Outreach Team and Enhancement Projects.

 

 

FISCAL IMPACT:

 

Amount: $84,999 (Street Outreach Team) and $256,913 (Enhancement Project)

 

Funding Source: 2008 HUD Supportive Housing Program

 

 

ADDITIONAL INFORMATION

 

The City Council approved the 2008 SHP application on September 22, 2008 by Roll Call No. 08-1660.  Included in the application were one year renewals of the Primary Health Care Street Outreach Team ($84,999) and the Enhancement Project ($256,913).

 

The Street Outreach Team consists of a licensed Mental Health Counselor and an Outreach Worker.  They make regular contact with homeless individuals who live on the streets, under bridges or in riverside camps.  They served 91 individuals in 2008 and 68% moved to either transitional or permanent housing.

 

The Enhancement Project is a combined initiative of case management, substance abuse assessment and referral, and health care for 148 homeless adults and children.
