additional work on the Southeast Connector – Des Moines River Bridge with
Levee, Sanitary Sewer, and Water Main Relocation for a total not to exceed

SYNOPSIS:
Recommend approval of Change
Order No. 5 in an amount not to exceed $78,400.30 for the construction of
structural concrete retaining walls with ornamental metal railings and Class 20
excavation with United
Contractors, Inc. and Subsidiaries, Steve Sandquist,
President, 6678 N.W. 62nd Avenue, Johnston, Iowa, 50131.

ADDITIONAL INFORMATION:
The
contract for the construction of the Southeast Connector Des Moines River
Bridge with Levee, Sanitary Sewer, and Water Main Relocation was awarded
to United Contractors, Inc. and Subsidiaries, in the
amount of $12,256,914.02. This
construction contract is to construct the Southeast Connector Dual Bridges over
the Des Moines River, along with related relocations.
Change
Order No. 5 compensates the contractor for 112.0 CY of wall excavation (Class 20
Excavation), 49.0 CY of structural concrete (Structural Concrete,
Miscellaneous), and 82.73 LF of metal railing (Ornamental Metal Railing), which
make up the four PCC retaining walls proposed for the outside bridge corners of
the east and westbound Southeast Connector Bridges over the Des Moines River.
These
retaining walls are necessary to retain the trail embankments that are
scheduled to be constructed on the side slopes of the earth levee. Construction of these retaining walls were
originally proposed to be part of the Southeast Connector Paving Project from
S.W. 2nd Street to S.E. 6th Street, but since these walls
are adjacent to the corner bridge monuments and are structural components, it
is believed to be more cost effective to have the bridge contractor complete
this work rather than include this work in the Southeast Connector road
project.
United
Contractors, Inc. and Subsidiaries agree to add the work addressed in Change
Order No. 5 to this contract by extending the contract bid item quantities and
honoring the April 2008 bid item pricing.
This project was bid using the Iowa DOT specifications which require that
the actual quantities of work completed be measured and paid for at the unit
prices included in the contract.
The
construction quantities and details of these additional retaining walls are
documented in two revised plan sheets (Plan File No. 481-002 and 481-129) as
well as five new plan sheets (Plan File No. 481‑275A through 481-275E).
This
change order adds a substantial amount of work to this contract. Because of this added work, a total of 40
additional working days are being added to this contract.
