First Steps

Step 1 – LAB RESULTS: Schedule an appointment with your personal health care provider or gather previous lab results using the Wellness Clinic Checklist.

Step 2 – PHA: Complete your online Personal Health Assessment (PHA) including your lab results and review your personalized report.

Step 3 – ENJOY: You will receive a $60 gift card for completing your Personal Health Assessment (PHA) by August 31. Spouses can receive a $30 gift card for completing their PHA by August 31. Gift cards will be disbursed after August 31.
