Biometric Screening Patent Result Sheet
Please take this form with you to your scheduled appointment to ensure that the appropriate biometric screenings are part of your routine physical. Then you can use the biometric values when completing the Personal Health Assessment.

Sample Personal Health Assessment (PHA)
This an example of the assessment that you will complete when you visit the online PHA tool at www.wellmark.com. All of the information that you provide will be kept strictly confidential.

Participant Report
This is an example of the report that you will receive after completing the PHA. It will provide you with information about your health risk factors and identify areas where you can make positive changes in your health - maybe you'll want to make a goal to walk more, manage your stress better, or quit smoking.

Aggregate Report
This is an example of the report that the City will receive after August 31st, when the PHA promotional period ends. The report will overview the health status of the City's workforce and assist the Wellness committee in its efforts to plan effective wellness activities and events.
